CHILDREN WITH KNOWN ALLERGIES (OR ALLERGIC REACTION POTENTIAL)
If your child has a known allergy. Please fill out the survey and medication administration form under the forms menu of this website. If your child may be in need of a medication during the school day. Please have these forms filled out and brought in by the start of the 2008-2009 school year. If your child may be in need of a medication during school or on the bus ride to and from school, please bring in the medication in the original packaging with your child’s prescription, on or before the first day of school.
If you are unsure if your child will require a medication during school or the bus ride to and from school, please consult with your child’s pediatrician.
