The Nurse’s Office takes this opportunity to acquaint you with the policies regarding immunization requirements, school physicals, sports physicals, and medication in school.
Immunization requirements are mandated by the State of New York. A list of immunization dates verified by a physician or a health record from a previous school must be presented to the School Nurse prior to entrance into school.
New York State requires that all 7th and 10th grade students be given a physical examination. Additionally, sports physicals are required for any student wishing to participate in a sport. Physicals are done by the School Physician during the regular school day and are arranged through the Nurse’s Office. Students who prefer to be examined by their own physician may obtain the appropriate forms from the School Nurse.
The New York State Department of Education has established protocols for the administration of medication during school hours. If you would like your child to receive over-the-counter medication such as Tylenol or Advil, a doctor’s note must be provided. If your child will require medication in school, please contact the Nurse’s Office for the required permission forms.
Note from physician
Please notify the Nurse’s Office of any major illnesses, injuries and/or any immunizations which your child may incur so that his/her health record is kept updated. If the illness or injury is such that it restricts gym participation, a note from the physician is required.
